To request a repair please complete and submit the below request:

  1. Enter plan number from purchase invoice.
  2. Add device serial numbers for the ones that need repair.   Also, list what the damage appears to be.  Example:  Broken Front Glass
  3. List all devices you need to have repaired, serviced, or replaced.
  4. Enter the contact name, contact email, and phone number plus extension for the Claim request.
    Enter the contact name, contact email, and phone number plus extension for the person directly handling the claim.
  5. Once you hit Enter, your claim request will be reviewed and once approved shipping will be arranged.  If you purchased a plan that included free Fed Ex shipping, your labels will be emailed to you shortly.  Please provide the email address to which the Fed Ex labels will be sent.

If you are going to box and ship the equipment to us, please send the box and a copy of this claim inside of the box to:
