To request a repair please complete and submit the below request:
- Enter plan number from purchase invoice.
- Add device serial numbers for the ones that need repair. Also, list what the damage appears to be. Example: Broken Front Glass
- List all devices you need to have repaired, serviced, or replaced.
- Enter the contact name, contact email, and phone number plus extension for the Claim request.

- Once you hit Enter, your claim request will be reviewed and once approved shipping will be arranged. If you purchased a plan that included free Fed Ex shipping, your labels will be emailed to you shortly. Please provide the email address to which the Fed Ex labels will be sent.
